      1. Any changes to your legal name need to be made with proper documentation by submitting a Legal Name Request Form

You can submit your unofficial Notre Dame transcript in your application materials and for a Ñý¼§Ö±²¥ State advisor to evaluate for course transfer and program applicability. However, Ñý¼§Ö±²¥ State must have your official transcript on file in order to process transfer credits to count toward program requirements and overall credit hours for a degree at Ñý¼§Ö±²¥ State.
